Please have ready a copy of the athlete’s current school physical and, for new members, a copy of their birth certificate.

This is parental consent for the treatment of minors in the case of illness or accident. Parental permission must be obtained before medical treatment can be rendered to any person less than eighteen years of age. The following consent form must be signed by the parent and/or guardian so that indicated care may be given without unnecessary delay. No major procedures will be performed except in an extreme emergency, without the parents being notified and fully informed. In the event that a parent does not want treatment rendered under any circumstances, the parent should cross out the word “GIVE” and circle the word “REFUSE.” This will Give/Refuse permission to the physician(s) at any physician’s office, hospital, or clinic to carry out emergency diagnostic and therapeutic procedures as may be necessary for my son/daughter and in the physician’s absence for the nurse on duty to render emergency care.

I understand that the training program will be $300.00 for the athlete(s) who are participating for half of a season (Indoor or Outdoor) and $425.00 for participating a full season (both Indoor & Outdoor). The club membership fee does not cover the cost of entry fees, transportation, food, or lodging when traveling. All fees paid are non-refundable and must be paid in cash, cashier’s check, personal check, or money order. There will be a return payment fee of $25 on each bad check. I agree to be responsible for these expenses.

In consideration of PEOPLE’S CHOICE, allowing my child/children to participate and compete with PEOPLE’S CHOICE, I intend to be legally bound for the child/children by executing this agreement. I hereby waive or give up any right to, or the child/children may have to, file a suit against the PEOPLE’S CHOICE, its coaches, parents, volunteers, area high schools, or any other person, organization, or entity assisting PEOPLE’S CHOICE.

I THE PARENT(S) AND ATHLETE HAVE REVIEWED THIS INFORMATION. We understand and agree that being a part of the People’s Choice team we commit to expectations of regular attendance, behavior acceptable to the Club, and good sportsmanship. Failure to abide by any of the above-stated Club expectations and respective parent and player obligations might result in the player’s suspension or termination at the discretion of the Coach.

I hereby give my consent to People’s Choice administrators, coaches, or volunteers to photograph, film, videotape, and then use, reproduce, and publish images of me and/or my child. I hereby release People’s Choice and his/her legal representatives and assigns from any, and all claims whatsoever in connection with the use, reproduction, and publication of the images thereof. I understand that if I choose not to have my child(s) pictures published in any way, I may request a release from People’s Choice that clarifies and formalizes my request. This applies as well to basic information about my child(s) which may be used in awards and other announcements made by the club, including Name, height, People’s Choice, grade, and track and field-related information like reach and vertical.

Marcia Thomas Enterprises (MTE) and People’s Choice have my permission to use my, and/or my child’s photograph publicly to promote People’s Choice and MTE. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use. I acknowledge this form is valid from signature until withdrawal, change in consent, or no consent involving the use of my child’s photograph.
